Above the medication pack gives a detailed description of the medication. Metformin which was stopped on 9-24-15 according to the medication sheet is white, round, 435 number id.
Below included in Keith's 10-25-15 medication packet was a metformin pill. So Keith had been taking Metformin a month after the medication sheet stated it had stopped.
Medications
Issues with medications... missing meds, meds signed off as administered but were not, medication being given a month after medication sheet discontinued medication. Staff going to use neosporin on a deep wound when doctors consultation sheet directed plain Vaseline with the word plain underlined. TideWay staff using "TideWay's diabetes kit" on multiple clients.

I take Keith's medications for the day with me during my visits. Staff put Keith's meds into either little plastic or paper cups or packets and then staples or tapes them shut.
