June 2012    During my visit, TideWay’s RN stated Keith had an appointment scheduled for June 7th with a dermatologist to check the growth on his head .   March 2012 I had UTMB scan in my guardianship paper with my name, phone number and address so that it would be in Keith’s file.   June 6th I contacted the dermatologist office and confirmed my contact information.    I specifically asked the Dermatologist check the growth on Keith’s left thigh.    I stated Keith has a brain injury with memory issues and will not remember so please be sure this information is passed along to the doctor.    I asked to be contacted with the results and was told it would take approximately one week.    June 14th  Dr Masel contacts me stating the growth on Keith’s thigh was cancerous and a procedure to remove was scheduled for June 21st.   I did not hear from the dermatologist Thurs 14th, Friday the 15th or Monday the 18th so on Monday the 18th I sent a fax stating Keith may reside at TideWay but neither TideWay nor Dr Masel have the authority to consent to a procedure for Keith.    On Tues 19th as I was flying to Texas, the dermatologist tried to contact me.    When I arrived, I took Keith to the clinic but the dermatologist had left the clinic for the day so an assistant addressed my questions and concerns.  


By the time I got home to NJ the next day, the dermatologist left me a message stating the June 21st appointment had been cancelled.


The next phone message was my lawyer saying urgent contact me immediately.  


I first called the dermatologist office giving consent and told them not to cancel Keith’s appointment and that TideWay will be bringing Keith to his appointment.    I then contacted the RN at TideWay and verified TideWay would see to it Keith was taken to his appointment.    I then called my lawyer and stated I confirmed Keith’s appointment with the dermatologist and TideWay will be taking Keith.  


In the meantime, Dr Masel had contacted Maureen McCutchen (TideWay’s lawyer) and Maggie Hindman (ad litem) stating I was endangering Keith’s welfare and I should be removed as guardian.

Dr Masel stating "His sister did not bring in any records".    I can't bring back records if I wasn't provided any to bring back.    I gave verbal instructions, written instructions and tried to contact by phone.

Notice the instructions have the word "plain" Vaseline underlined twice.

Phone call to Anna TideWay's RN to confirm instructions but she never returned my call

No written instructions provided by dermatologist so I gave verbal instructions to staff and left Anna TideWay's RN a written note.

Dermatology

Statement by Dr Masel..."we have to coordinate her visits with dermatology clinic visits".    TideWay/Dr Masel coordinate nothing in regards to my visits or my being with Keith at doctor appointments.   I make and take Keith to his appointments.    If I am aware of an appointment made by TideWay I sometimes try to schedule my visit on Keith's appointment date.    I made Keith's appointment with GI and took Keith to his colonoscopy.    I made the apoointment with the dermatologist and took Keith.     Taking and meeting Keith's doctors in person is the only way I am acknowledged as Keith's guardian.

Sept 8, 2014 I took Keith to Dr Kelly, dermatologist, to remove cancer growth.   This removal involve a deep chunk of flesh being removed from Keith’s leg requiring stitches.    I returned the consultation sheet to TideWay with instructions.    Note the instructions indicate to apply ‘plain’ vaseline.   The word ‘plain’ is underlined twice.    TideWay staff told me they were going to apply Neosporin antibiotic ointment.   I said no you are not.   The instructions clearly state ‘plain’ vaseline.   TideWay staff says to me, "we don’t have any vaseline."   I said Keith has a jar of vaseline in his room.   Just like everything else, Keith’s jar of vaseline was gone, so then I told staff after my visit time is up at 9pm, I will go to Walmart and buy Keith vaseline.    Keith was waiting at the door for me when I returned with the Vaseline.

I went and bought plain Vaseline for Keith to put on his wound as directed by the dermatologist.

Keith and I returned to TideWay on Feb 20, 2013 to meet up with Dr Masel while he was making his monthly round at TideWay.

I requested TideWay take Keith to the dermatologist.  The appointment scheduled by TideWay was months away in July.  The Dermatologist office contacted me and rescheduled for May but since I didn't request TideWay take Keith until ten days prior to his May appointment TideWay stated they were unable to get Keith to the May appointment so it was rescheduled for June.   In June Keith finally got to the dermatologist to remove the growth from the side of his face.    When I want Keith taken to the dermatologist for a growth removed it isn't urgent but when Dr Masel wants to make it appear I am endangering Keith's welfare the growth removal is urgent.
